DOL Model COBRA Continuation Coverage General Notice
Employers must provide the COBRA General Notice to each covered employee and spouse (if any) at the time coverage begins under the health plan. On May 2, 2014, the DOL updated the model notice to provide information on coverage options under the health insurance Exchanges (or Marketplaces). To properly use the model notice, the plan administrator must fill in the blanks with plan information.

Download the Department of Labor's model COBRA Election Notice. It describes a qualified beneficiary's right to COBRA coverage and explains how to make an election. Model Employer CHIP Notice
This model notice may be used to satisfy the employer notice requirement under the Children's Health Insurance Program Reauthorization Act of 2009 (CHIPRA). CHIPRA created notice and disclosure obligations for employers that maintain group health plans in states that provide Medicaid or state children's health insurance program (CHIP) assistance in the form of premium assistance subsidies. The DOL updated this notice on May 2, 2014, to include information on the health insurance Marketplaces.
